If anxiety is affecting how you feel in bed, you are not imagining the link: anxiety can interfere with erections, and that is common and treatable. DirectMax is a prescription ED product that does not treat anxiety itself; its maker designed it to support the erection response with a sildenafil, tadalafil and apomorphine tablet that dissolves under the tongue. For scale, the FDA-posted Cialis prescribing information describes 22 tadalafil trials in over 4,000 patients, each up to 24 weeks long, that evaluated erectile function.
DirectMax is a prescription tablet built around three ingredients
DirectMax is a compounded prescription product and is not FDA-approved. Direct Meds, which sells it, lists sildenafil, tadalafil and apomorphine as its three active ingredients on its DirectMax page (viewed November 2, 2026). The seller’s product graphics describe apomorphine as “primes for arousal,” sildenafil as “maintains blood flow” and tadalafil as “extends effectiveness.”
The seller describes DirectMax as physician-designed and says licensed physicians customize the dose to your body, goals and medical profile. It is pitched at people who tried a single-ingredient pill and want another prescription option. If anxiety or low mood has dimmed desire as well as erections, note that MedlinePlus says tadalafil does not increase sexual desire. The seller’s claim for apomorphine is the arousal piece.

Anxiety, low mood and ED often show up together
A 2016 review in Nature Reviews Disease Primers describes ED as having organic, relational and psychological components, and says most patients have symptoms of depression and anxiety related to sexual performance. That answers the question of whether performance anxiety can go with ED: yes, it is a recognized part of the picture. The same review says ED is strongly associated with metabolic syndrome and cardiovascular disease, so cardiac assessment may be warranted in men with ED symptoms.
A 2000 review in Postgraduate Medicine found the link between depression and ED is firmly established, though in one person it may be difficult to tell whether ED causes the depression or the reverse. A 2007 review in Urologic Clinics of North America agrees the two are clearly associated.
What is the difference between psychological and physical ED?
Psychological ED is tied to mood, stress or worry about performance. Physical ED comes from the body: MedlinePlus says ED can be a sign of health problems, such as clogged blood vessels or nerve damage from diabetes. Many people have a mix. NIDDK says clinicians diagnose ED with a medical, sexual and mental health history, a physical exam, and lab and other tests. That is how the two get sorted, not by a symptom alone.
Why am I only semi-hard, or not “rock hard” anymore?
NIDDK defines ED as trouble getting or keeping an erection firm enough for sex. It lists getting an erection only sometimes, or one that does not last long enough, as ED symptoms. A health care professional diagnoses ED with a medical, sexual and mental health history, a physical exam, and lab and other tests. Worry can sit on top of any of these changes, and a full history is what separates them.
Antidepressants and anxiety medicines can affect erections
If you take medicine for anxiety or low mood, it may be part of the story. MedlinePlus notes providers may prescribe antidepressants for anxiety as well as depression. A 2023 clinical review in the British Journal of Nursing says sexual side effects of psychotropic medication are very common, cause distress and can persist after the medication is withdrawn.
A 2006 German review found ED to be one of the principal reasons for stopping antidepressant treatment, and noted that avoiding or switching antidepressants can bring a rise in depressive symptoms. The same review concluded oral phosphodiesterase inhibitors, the drug class that includes sildenafil and tadalafil, are an option for ED resulting from depression and from antidepressant medication. A registered 30-person trial (15 men, 15 women) planned to switch people with SSRI-related sexual side effects to another medicine over a 10-week open-label follow-up; the registration describes the design, not a result.
Do Viagra and similar tablets help with performance anxiety?
They are studied for erections, not for anxiety. The studies cited here are on sildenafil and tadalafil tablets, not on DirectMax itself. In the Cialis label’s seven 12-week, randomized, placebo-controlled studies, outcomes were measured with the International Index of Erectile Function and Sexual Encounter Profile questions, and the label says tadalafil was shown to be effective in improving erectile function.
A registered trial of 100 planned participants, with its status listed as unknown, was designed to test whether a stress-reduction phone app helps men with ED who take PDE5 inhibitors and have performance anxiety. It is a registration, not a result.
What fits alongside a tablet when anxiety is the driver
NIDDK says clinicians treat the underlying causes of ED first and then focus on improving sexual function, and that a healthy diet can lower the risk of ED or improve symptoms. MedlinePlus adds that more exercise, weight loss or quitting smoking may also help. If the worry itself is what you want to work on, our guides to mental health supplements and natural anxiety relief supplements lay out what the research on non-prescription options shows.

Safety: nitrates, alpha-blockers, heart conditions and emergency signs
These warnings come from Direct Meds’ safety notice and from the tadalafil and sildenafil labels. They matter more if worry has you eager to try something quickly.
| Issue | What the sources say |
|---|---|
| Nitrates | Direct Meds says not to use ED medications with nitrates for chest pain or guanylate cyclase stimulators such as riociguat, because of a dangerous drop in blood pressure. |
| Alpha-blockers | The Cialis label says both drug classes lower blood pressure, and together can lower it significantly, which may lead to fainting. |
| Heart conditions | Direct Meds lists heart problems, low blood pressure, and recent heart attack or stroke. MedlinePlus says sexual activity can strain the heart; chest pain, dizziness or nausea during sex means calling a doctor right away. |
| Erection over 4 hours (priapism) | The Cialis label says an erection lasting more than 4 hours, painful or not, needs emergency care. |
| Sudden vision loss | The Cialis label says to stop PDE5 inhibitors and seek care; loss of vision has been reported rarely after approval. |
| Sudden hearing loss | MedlinePlus says to call a doctor immediately and take no more doses of sildenafil, tadalafil or similar medicines until you do. |
| Common side effects | Direct Meds lists headache, flushing, nasal congestion, mild indigestion, dizziness, back or muscle discomfort, nausea, temporary vision changes and mild irritation under the tongue. In the Viagra label’s studies (734 men on Viagra, 725 on placebo), vision changes were mild and transient. |

Your next step if anxiety and ED are tangled together
Bring your anxiety, your mood, any antidepressant and your erection changes to one clinician, so they can look at the whole picture and decide on your medication with you. That conversation is how the physical and psychological pieces get sorted, and it keeps every choice, including DirectMax, yours to make.
